Um die Bildung von ad-hoc Selbsthilfegruppen und reale Treffen zu f\u00f6rdern, k\u00f6nnen sich die Teilnehmenden digital vernetzen. Gemeinsames Training ist nicht nur motivierend, sondern f\u00fchrt auch zur Verringerung von Einsamkeit.<\/p>\n<p>Der SmartCOPDtrainer soll PatientInnen unterst\u00fctzen, Krisen (sogenannte Exazerbationen) signifikant zu reduzieren.<\/p>\n<p>Die Entwicklung des SmartCOPDtrainers folgt der ISO 9241-210 Norm der \u201eMenschzentrierten Entwicklung\u201c. Das bedeutet die Anforderungen, W\u00fcnsche, \u00c4ngste und Sorgen der PatientInnen sowie deren Erhebung stehen im Vordergrund, und nicht die technologischen M\u00f6glichkeiten. PatientInnen, \u00c4rztInnen und FachexpertInnen sind in allen Phasen des Projekts mit eingebunden. Das erlaubt uns eine wissenschaftliche Effektkontrolle sowie die iterative Anpassung der Ans\u00e4tze auf Basis des Feedbacks.<\/p>\n<\/div><\/section><br \/>\n<div class=\"togglecontainer   toggle_close_all  \" >\n<section class=\"av_toggle_section\"  itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/CreativeWork\"  >    <div class=\"single_toggle\" data-tags=\"{Alle} \"  >        <p data-fake-id=\"#toggle-id-1\" class=\"toggler \"  itemprop=\"headline\"  >Forschungsfragen<span class=\"toggle_icon\" >        <span class=\"vert_icon\"><\/span><span class=\"hor_icon\"><\/span><\/span><\/p>        <div id=\"toggle-id-1-container\" class=\"toggle_wrap \"  >            <div class=\"toggle_content invers-color \"  itemprop=\"text\"   ><ul>\n<li>Wie gut k\u00f6nnen COPD-PatientInnen mit Hilfe eines pers\u00f6nlichen digitalen Assistenten angeleitet werden um Exazerbationen rechtzeitig zu erkennen und zu verhindern?<\/li>\n<li>Welche persuasiven Strategien k\u00f6nnen helfen die PatientInnen zu einer intensiven und selbstverantwortlichen Therapie (Patienten Compliance) in Einklang mit den Empfehlungen des Arztes bzw. der \u00c4rztin zu bewegen?<\/li>\n<li>Welche technischen Ma\u00dfnahmen sind geeignet die PatientInnen in ihrem Alltag effektiv zu unterst\u00fctzen?<\/li>\n<\/ul>\n            <\/div>        <\/div>    <\/div><\/section>\n<section class=\"av_toggle_section\"  itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/CreativeWork\"  >    <div class=\"single_toggle\" data-tags=\"{Alle} \"  >        <p data-fake-id=\"#toggle-id-2\" class=\"toggler \"  itemprop=\"headline\"  >Ziele<span class=\"toggle_icon\" >        <span class=\"vert_icon\"><\/span><span class=\"hor_icon\"><\/span><\/span><\/p>        <div id=\"toggle-id-2-container\" class=\"toggle_wrap \"  >            <div class=\"toggle_content invers-color \"  itemprop=\"text\"   ><h4>Vermeidung von Exazerbationen<\/h4>\n<p>Eine Studie von Federico Fiorentino aus Palma de Mallorca, Spanien, zeigt, dass COPD-PatientInnen mit durchschnittlich zwei oder mehr Exazerbationen pro Jahr, durch eine konsequente PatientInnenschulung, Ern\u00e4hrungsanalyse und -beratung, Betreuung durch einen Fallmanager, sowie Ma\u00dfnahmen zur F\u00f6rderung der fr\u00fchen Behandlung bei beginnender Exazerbation, die Anzahl der Krankenhausaufenthalte auf 0,27 pro Jahr reduziert und auch die Zahl der im Krankenhaus verbrachten Tage sich von 37 auf 2 Tage reduzierte. Allerdings war bei dieser Studie ein hoher personeller Betreuungsaufwand notwendig.<\/p>\n<p>Der SmartCOPDtrainer als digitaler Assistent soll einen \u00e4hnlichen Effekt ohne personellen Betreuungsaufwand erreichen. Dazu gibt es folgende Ansatzpunkte:<\/p>\n<ul>\n<li>PatientInnen m\u00fcssen die Diagnose akzeptieren und ihre Krankheit verstehen.<\/li>\n<li>Der Einfluss von \u00e4u\u00dferen Faktoren, wie Umweltbedingungen und Lebensweise m\u00fcssen den PatientInnen bewusst sein. Dazu geh\u00f6rt auch, \u00fcber die Faktoren Bescheid zu wissen, die den Zustand verschlimmern.<\/li>\n<li>Self-monitoring deuten und sch\u00e4tzen lernen.<\/li>\n<li>Verschlechterungen im Atemverhalten erkennen lernen und entsprechend reagieren k\u00f6nnen.<\/li>\n<li>K\u00f6rperliche Bet\u00e4tigung und Training: Muskelaufbau, Steigerung der Kondition und des Immunsystems reduzieren Luftnot und die Anf\u00e4lligkeit f\u00fcr Infekte, und erleichtern den COPD-PatientInnen, mit ihrer Krankheit zu leben. Voraussetzung ist allerdings, dass diese sportliche Bet\u00e4tigung regelm\u00e4\u00dfig durchgef\u00fchrt wird.<\/li>\n<li>Rechtzeitige Erinnerung an Impfungen, wie beispielsweise gegen Grippe und Pneumokokken.<\/li>\n<li>Konsequente medikament\u00f6se Behandlung, bei gleichzeitiger Aufkl\u00e4rung \u00fcber die Wirkung und Nebenwirkung. Z.B. kann ICS (inhalierbares Kortikosteroid) bei l\u00e4ngerer Therapie zu Pneumonien und Osteoporose f\u00fchren.<\/li>\n<li>PatientInnen muss klar werden, unter welchen Bedingungen sie dringend medizinische Hilfe ben\u00f6tigen w\u00fcrden, bevor eine Exazerbation eintritt. Werden entstehende Krisen rechtzeitig erkannt, k\u00f6nnen Ma\u00dfnahmen zur Vermeidung gesetzt werden.<\/li>\n<\/ul>\n<h3>Persuasive Strategien<\/h3>\n<p>Persuasive Strategien bzw. Technologien haben das Ziel NutzerInnen bei einer gew\u00fcnschten Verhaltens\u00e4nderung zu unterst\u00fctzen. In SmartCOPDtrainer k\u00f6nnen PatientInnen durch persuasive Strategien dauerhaft zur Nutzung des digitalen Assistenten motiviert werden, was sich wiederum in einer besseren Patienten Adherence (Therapiebefolgung) niederschl\u00e4gt.
